When you come to pick up your medical records, please bring photo ID. If someone other than the patient is picking up the records, that person needs photo ID and an original signed authorization letter from the patient. There may be a charge for creating a copy of your records.

My Health Rec
My Health Rec allows you to view, download and transmit your child’s up-to-date health information, 24 hours a day, seven days a week.
With My Health Rec you can:
Privately view some lab results
Access health records
Download health records to share with physicians
Pay health bills online
An in-person visit to Palm Beach Children’s Hospital is required for enrollment in My Health Rec. Once you have enrolled, you will be able to access your health records online at any time.
